Background: Uterus didelphys is a rare type of uterine malformation with poor obstetric outcome. Uterus didelphys, particularly when associated with ectopic gestation, is an uncommon condition. A patient in which this situation existed has been reported. There should be a high degree of suspicion for proper management of such cases as these patients carry great morbidity and mortality if left undiagnosed and untreated. Case presentation: 34-year-old woman, G3P1L1A1 with 3 month amenorrhea with spotting pv came in emergency. Ultrsonography suggestive of didelphic uterus with ruptured left ectopic pregnancy. Emergency laparotomy with left salpingectomy done. Conclusion: Didelphic uterus still remains a diagnostic and therapeutic challenge when such patients come in acute emergency, with timely and proper intervention, life of the patient can be saved.
