Nutcracker syndrome (CNS) is characterized by compression of the left renal vein (LRV) between the abdominal aorta and the superior mesenteric artery (SMA) due to a reduction in the aortic-mesenteric angle. Stenosis of the ureteropelvic junction (UPJ) occurs due to obstruction of urinary flow at the anatomical confluence of the proximal ureter with the renal pelvis, due to intrinsic or extrinsic forces, and may be congenital or acquired. We will describe the case of a 52-year-old woman, without previous comorbidities, complaining of pain in the left abdominal flank, nausea and sudden onset vomiting. During the investigation, after carrying out several tests, including tomography and scintigraphy, it was observed that the patient presents a rare association of these two pathologies, with few cases reported in the literature.
