The present work aims to be a reflexive analysis on the chronic conditions (MACC) model of attention as the organizing axis of primary health care teams with transversality in psychosocial attention. This is a descriptive study with reflexive analysis. The results were discussed in three categories: MACC as the guiding principle; Psychosocial Attention Network and MACC; the relevance of MACC in the training of health professionals. As results, for primary care to work integrated psychosocial care is necessary a reorientation of the model adopted in the network points. Using the model of attention to chronic conditions, demystifying the view focused only on diseases and coping with acute and sporadic, implies serving the user with a vision of integrality of care and not based on a pathology or symptomatological complaint, but in the individual context, familiar and social, that the user is inserted. Thus, it is necessary to reflect, understand and re-significate learning in the training of health professionals.
