The absence of diagnostic tools to determine the maturity level of Knowledge Management in small and medium media is detected. The level of maturity, for use, use of an instrument for data collection. Each data collection allows to diagnose or measure the use of tools and practices in the Processes, Technology and People Knowledge Management dimensions. Following a sequence of these diagnoses, one can then determine the level of maturity of this organization. This article aims to present a data collection instrument that allows diagnosing or measuring the use of tools and practices in the Processes, Technology and People dimensions of Knowledge Management. The methodology is applied in nature with a quantitative approach through a single case study in an elementary school in the city of Maringá, state of Paraná. An online questionnaire was built from the literature review and IPEA research. It was found that the school case study recorded a general use grade of 53.91%. The school should seek to maintain the results achieved in the People and Technology dimensions, and endeavor to improve the outcome of the Processes dimension.
